This application is requested to determine your eligibility to participate in the Career Pathways program at Ozarka College.  Completing this form does not commit you to participate in the program.

Career Pathways will loan textbooks for core college classes each semester to eligible Career Pathways students.  The purpose of the book loan program is to help students with the increasing cost of books by providing books that can be checked out for the length of the course and returned, all at no cost to the student.  Completion of this form does not guarantee that you will receive services through the book loan library.  Books will be awarded to students based on availability.

In an effort to more accurately serve a larger population of Career Pathways students at Ozarka College with expenses related to reaching your educational goals, we ask that you rank the services in the order that you would most prefer them.  This information is requested to help us determine the services that you are eligible for and would benefit you most.  (Completing this form does not mean that you will receive the service.) 

Students will be asked to turn in a monthly employment verification form to the Career Pathways office throughout the semester.  Students who do not turn in forms will not be considered for future financial services.
